Dead Bees - Inner Cover

This is the first year for us doing bee keeping. We have 2 hives. We added our second hive box about a week ago. When I opened the outer cover, there were a number of dead bees on top of the inner cover. This was also the case for the second hive.

Would anyone have a suggestion as to what is going on or what is causing this?

They get lost up there and die. Just put a bit of flywire over the hole so they can’t get up yhere.
